I had this issue for many months. It appears that a driver for CSR Harmony Bluetooth is the culprit. The simple fix is:
Navigate to C:\Program Files\CSR\CSR Harmony Wireless Software Stack
and rename
BLEtokenCredentialProvider.dll
to
BLEtokenCredentialProvider.dll.bad
